Your recordable rate looks fine. Is your company exposed to a fatality?
The problem
Boards, insurers and owners are asking a harder question than “how many injuries?” They want to know whether a serious injury is building in the system — and most dashboards can’t answer it.
High-energy near misses get coded the same as paper cuts. Corrective actions close with “toolbox talk delivered” and the same event repeats eight months later. Critical controls are verified on paper, if at all. Hazard assessments get copied forward until they stop meaning anything.
What you get each quarter
A board-ready report, plus a 60-minute walkthrough with your leadership team, covering:
- Serious-injury precursors — events with high-energy potential your recordable rate is hiding
- Corrective-action effectiveness — which closed actions actually worked, and which events are repeating
- Critical-control verification — which controls are being checked in the field, and which aren’t
- Hazard-assessment quality — where assessments are being copied rather than done
- IOSS Stability Score — one trend the board can track each quarter (the full score needs control-verification data; I’ll help you start capturing it)
- Three priority actions for the next quarter
How it works
- You export your incident, near-miss, hazard-assessment and corrective-action records from your existing system, with names and personal details removed — no new software
- I classify every event for serious-injury potential using a consistent energy-based method
- You receive the report and walkthrough within three weeks of the data cut-off
AI-assisted analysis lets me review every record, not a sample. All findings are reviewed by me. I work under an NDA, use de-identified data only, and never share or pool it across clients.
What I need from you: one data export per quarter and 60 minutes for the walkthrough.
Who it’s for
Safety leaders, CEOs and boards at mid-sized contractors and asset owners in high-risk industrial work — especially those whose board or insurer has started asking questions their current reports can’t answer.
Founding-client rates
Founding-client rates are available for the first three clients, including an optional one-time baseline review. Book a fit call to discuss scope.
What did your board last ask about safety that you couldn’t answer with data? Let’s start there.
